Initiating Therapy

My child has come a long way since we've received services at Kid Talk.

Our therapist makes it fun for our son, he doesn't know his is working.

Please complete all the steps below to successfully initiate services with Kid Talk

  1. Contact your insurance company. Download and complete Medical Insurance Benefits Worksheet while talking with your insurance company.
  2. Obtain order/referral from your child’s physician if you haven’t already. Your physician can send us an order/referral via our MD Order/Referrals Page.
  3. Call Kid Talk at 952-443-9888 to set up an appointment.
  4. Complete the appropriate case history form and either email, fax or mail the form back to Kid Talk. Fax: 952-443-9804, PO Box 34, Victoria, MN 55386. Your case history and any other requested information must be received no later than one week before you scheduled appointment (Feeding Case History Forms along with a video or DVD of your child eating a typical meal must be returned 3 weeks prior). If Kid Talk does not receive this information as stated above your appointment will need to be rescheduled.

  5. Read, sign and return the following at the time of your evaluation:

    Patient Service Agreement
    Consent to Treat
  6. HIPAA Privacy Statement
